Fife Health and Social Care Partnership (the H&SCP) requires to commission personalised and outcome-based day care services from external care providers for adults and older people aged 16 years upwards, with a range of support needs outlined in the Schedule.The aim of the service is to provide a safe and stimulating environment for people to be supported during the day, sustaining and/or developing physical, sensory, intellectual, and social abilities by providing individual, intensive, comprehensive personal plans underpinned by a person-centred planning and outcome focused approach.
