The establishment of a Community Link Worker and Wellbeing Practitioner Service will support adults (aged 18 to 65 years) with moderate Mental Health problems to manage their health and wellbeing. Support will be person-centred and delivered via two community hubs – one in the refurbished building in the grounds of St John Hospital in Livingston, and the other in Boghall Clinc in Bathgate, and in all GP Practices in West Lothian for a defined number of sessions per week. In the first instance, support will be available Monday to Friday between the hours of 9am and 5pm, although this may alter depending on the demand for the service. Some out-of-hours work is likely. As this is a new Service, some flexibility of the Provider’s part is expected.
