Conwy County Borough Council is considering an opportunity to set up a peripatetic service as part of business continuity to respond to urgent demands for care and support in the Conwy area. The service will include provision of personal care and support to individuals living in their own home within the Conwy area. Intentions are for this team to work closely with the Council’s in-house service to enable the re-ablement team to pick up new care packages as part of hospital discharges and respond to urgent requests.
