These networks work to share best practices, develop care standards, and improve the quality of diabetes care. <br/>The NHS Diabetes Prevention Programme Identifies individuals at risk of type 2 diabetes based on blood test results (HbA1c and fasting plasma glucose) or previous gestational diabetes, it provides a tailored, nine-month, evidence-based lifestyle change program to help people reduce their risk
