The service is available to patients aged 18 and over with a registered GP in the SNEE area.

This service sits within the stroke and neuro rehabilitation suite of services to help support those who are suffering the affects of a stroke.

Comprehensive Support for Stroke Survivors and Carers:

Extends support from the acute stroke unit to one year post-stroke, or longer based on patient needs.
Provides a single point of contact for stroke survivors and carers to navigate the health and social care system effectively.
Includes independent needs assessments and ongoing support for patients and carers.
