The aim of the Service is to build the resilience of Carers (of people with Dementia) to maintain their caring role for as long as they choose and thereby reduce inappropriate admissions to hospital or premature admission to long term Residential Care.<br/><br/>The Contract will be for a term of two years with the opportunity to extend for one year and a further one year subject to mutual agreement between the successful Bidder and the Authority.
