Delivery of NHS Health Checks. Providers will be responsible for inviting the eligible population, providing NHS Health Checks and offering clinically appropriate lifestyle advice, risk factor management and medication. The detailed requirements are defined in the
Specification.
Providers can bid for the contract as individual practices or groups of practices/partnerships. Where more than one practice bids then there should be an agreed representative who shall ensure that all practices meet the requirements of the contract and who shall support and manage providers to do this.
All General Practices wishing to deliver NHS Health Checks will need to apply to join the Framework.
