Description

Call off from Crown Commercial Services G-Cloud 13 Framework.

In line with the steer from NHS England, the commissioner is seeking to offer patient choice of provision to support improved uptake of structured education, by commissioning a remote, digitally-enabled structured education service, Oviva Diabetes Support.

Objectives

The aims and objectives of service are:

Provide Oviva Diabetes Support to people with type 2 diabetes registered with a GP practice in Buckinghamshire CCG as an alternative to face-toface education.
Support participants to increase confidence in self-managing their condition.
Support participants to make lifestyle changes that offer benefits for their long-term health, including improving HbA1c and weight loss, aligned to personal goals.
Support reductions in diabetes and blood pressure medication use.
Support reductions in use of primary and secondary care diabetes services.
Support effective behaviour change through the use of evidence based behaviour change approaches in coaching, educational content and digital technology.
