Provision of a Remote Care Monitoring Solution to deploy remote diagnostics across 8-10 care homes for a period of one year from the start of the contract. Across the usage period we wish to test the impact of the solution on the following:
1. Patient outcomes and experience.
2. Improvements in GP ability to prevent call out of emergency services.
3. Reductions in unnecessary conveyance to hospital.
4. Reductions in emergency admissions.
5. Demonstrate the usability of the solution for wider health care professionals to support patients.
6. Create an evidence base for the wider scale and deployment of the solution.
