This notice follows the publication of a Voluntary Ex Ante Transparency Notice published in the Find A Tender service reference number 2021/S 000-031536.
The service is to deliver Continuing Healthcare Services to patients in Nursing Homes commissioned by West Essex Clinical Commissioning Group. The service supports people with long-term complex health needs who have been assessed, and qualify for, NHS funding. The service will be delivered against the local specification which is aligned to the 11 domains of the Decision Support Tool.

The contract is being awarded to several nursing homes within the West Essex area and will be for a period of 12 months with the option to extend for a further 12 months.
