Hertfordshire County Council is carrying out Pre-Procurement Market Engagement (PPME) in relation to the Hospital and Community Navigation Service (HCNS) which is a social prescribing service that currently supports adults throughout Hertfordshire to build resilience, maintain independence, improve health and wellbeing and enable them to connect to their local community.
