BHR CCGs wish to commission 22 beds across 2 or more Nursing Homes within the Borough of Havering.
The pilot D2A process will actively support the Home First model; returning home will always be considered as part of the discharge decision making process, only where this is not possible will D2A be considered.
The D2A pathway is an interim placement rather than a longer-term placement and this will be communicated effectively to patients and carers/family members throughout the process.
Referrals will be made onto the pathway via the trusted assessor approach to the CCG team who will then arrange the placement in one of the commissioned nursing homes.

At the Commissioner's discretion, only those providers who evidence that they meet the above requirements may be taken through to the next round of the procurement process.
