Direct Award to deliver short term support within a supported accommodation setting to vulnerable adults (over 18) who are at risk of becoming homeless.
The service will work closely with other homeless prevention services commissioned by Sandwell Council including Supported Housing for Single Adults services, SWEP and Housing First etc to reduce homelessness within the Borough
The Contractor shall work with Service Users, offering support to overcome barriers and develop skills which will enable them to source and sustain a permanent tenancy of their own in the following key areas and will be the basis for outcomes monitoring of the service:
