The Provider Services Team are responsible for providing homecare services through an internal team of staff, focusing on reablement care, hospital discharges and short-term packages. The current Homecare management system, CM2000 is coming to the end of contract in November 2021. Prospective suppliers are invited to respond with a detailed quote for the provision of a solution for the next 3-5 years (3 years with option to extend by a further 2 years).
