The service will provide:
⢠An assessment and rehabilitation service offering advice, training and emotional support
⢠A floating support service to named people in their own homes
The floating support service is delivered as part of the Housing Related Support Provision. The main purpose is to develop and maintain a person's ability to live independently in their own home.
The service will be provided to a minimum of 30 people living anywhere in the county at any one time.
Services will be provided in line with the outcomes of the UK Vision Strategy, Vision 2020.
