NHS Derby and Derbyshire CCG seeks competitive bids for the Provision of Primary Care Network Development
The aim of the support is to provide developmental support to Derbyshire's 15 PCNs and their respective CDs, whilst helping Derbyshire to take a system wide approach to PCN development.
The intended audience is twofold:
1. PCN Clinical Directors - to provide tailored support to understand their current position and assist them to begin their development journey as PCNs
2. System level support including the CCG and GP Task Force - to provide support which joins up the needs and priorities of the individual PCNs as a whole, and helps Derbyshire to coordinate and move forward in a coherent and systematic way
The overall objectives include:
⢠Provide development support to PCNs and their CDs
⢠Design the proposed support to be tailored to the needs of individual PCNs/ CDs
⢠Work with CDs in a way which reflects their constraints and makes best use of their limited time
⢠Build upon and avoid duplication of effort and outputs already achieved
⢠Assist CDs in understanding their current situation and support them to plan their next steps in the most effective manner
⢠Work cross-system to ensure that on-going development and support needs are understood across Derbyshire
⢠Work with local support teams to build a common structured approach to PCN development.
