East Berkshire Clinical Commissioning Group ("EBCCG") are seeking expressions of interest from potential providers of a Social Prescribing Service across the Royal Borough of Windsor and Maidenhead.
The Social Prescribing Service is a single point of access that identifies and supports eligible patients with multiple social, emotional and practical needs.
The aims and objectives of the service are to:
⢠Improve wellbeing and ability to self-care.
⢠Reduce inappropriate appointments and use of other services.
⢠Increase the number of patients.
⢠Actively identify individuals in the specified categories who will benefit from the service.
The successful provider will be expected to:
⢠Triage patients to Link Workers.
⢠Provide assessment of needs i.e. motivational support, goal-setting and referrals to other suitable services.
⢠Develop a Personal Care Plan (PCP).
⢠Provide consultation, wrap around support for patients i.e. be-friending, activities, exercise classes, benefits assessments, housing, employment, training, arts, creative activities, emotional support etc.
⢠Refer patients to relevant and appropriate services.
⢠Regularly review patients and PCPs
The contract value is estimated at £140,000 per annum, with a contract term of 2 years plus 1 year optional extension with the service commencing on 1st April 2021.
This procurement is being carried out by NHS South, Central and West Commissioning Support Unit (SCW) on behalf of the Commissioners.
