The framework will provide social care funded packages of support to adults aged 18 and over, who meet the eligibility criteria. This includes adults who have assessed care and support needs associated with older age, physical and sensory impairment, mental illness, learning disability and for people living with dementia.
Adults relevant to both levels of Service include:
· Older People, including older people experiencing mental and emotional distress and with a diagnosis of Dementia.
· People with a physical disability and sensory impairment, including people living with long term conditions.
· People experiencing mental or emotional distress.
· People with a learning disability.
· People with Autistic Spectrum Disorders (ASD)
· People living with HIV/Aids.
· People with end-of-life care needs (i.e. in last months, weeks and days of life).
· Vulnerable adults
Initial applications to be on the Framework will be 09/09/2022. The Framework will start on 1st November 2022 and end on 30th October 2024 with options to extend for 3 further periods each of a year. Applications to go onto the Framework can be made at any time and will be evaluated after 30th of each month.
