This contract is for the provision of specialist services for young people and adults with a
Learning Disability including Autism and/or Mental Health care and support needs, whether funded under social care legislation, as NHS Continuing Health Care, or as aftercare under
Section 117 of the Mental Health Act. The aim of the Service is to support Service Users to live independently in the home of their choice for as long as possible and to enable them to lead a good quality of life. This Flexible Purchasing Solution will comprise the following three
