The overall purpose of the service is to provide a Carers Hub on behalf of Worcestershire County Council. The Provider manages and co-ordinates support for adult Carers (including parent carers of adults and young adults) across the county of Worcestershire. This is achieved by the Hub providing one point of contact for all adult Carers. The Hub co-ordinates and improves access to local support. The Hub covers the full range of information, advice and support that Carers may require through their caring journey and beyond. And signposts when required to more specialist support. The Carer assessment, reviews and support is integrated with the work of Adult Social Care Social Work teams, including the Young Adults Team for parent Carers and kinship Carers. The provider will have access to supporting documents and systems.
